The 'Cow Level' is one of the Diablo series' long-standing Easter eggs, and players have made various efforts to find it in Diablo 4 as well. With the new expansion, 'Vessel of Hatred', you can finally enter the Cow Level. I would like to share how to enter the Cow Level quest and my review after completing it myself.
To enter the Cow Level in Diablo 4, you must place three materials—Endurance Potion, Rusted Bardiche, and Neyrelle's Hand—into the Horadric Chest to obtain the materials needed to travel to Sailara Island. Please note that entering the Cow Level from Sailara Island is only possible from 9:00 AM Tuesday to 8:59 AM Wednesday (KST), so be sure to plan accordingly.

First material, Endurance Potion
1. Acquire three types of relics by eliminating 666 cows
2. Drop the three relics at the Ked Bardu pond and pick them up again
3. Interact with the cow statues at the Ked Bardu pond to obtain the Strange Key
4. Enter the Lonely Shack at the eastern end of Scosglen
5. Eliminate the cows inside the Lonely Shack to obtain the Endurance Potion
The first step is to eliminate cows located throughout Sanctuary. This is the most time-consuming part of the entry process. In my case, it took about 6–7 hours. You need to obtain the Complex Metal Shard, Bloody Wooden Shard, and Musty Tome, which symbolize the materials for the Cow Level in Diablo 2. These three items can be obtained from cows that respawn in Dry Steppes, Hawezar, and Scosglen, respectively.
To get the first item, you must eliminate a total of 666 cows. I recommend starting in Farobru in the Dry Steppes, as the path is clean and it has the highest density of cows. After repeatedly logging out and in to respawn cows and eliminating 666 of them in Farobru to obtain the Complex Metal Shard, items will drop with a certain probability whenever you eliminate cows thereafter.
If you are unlucky or want to avoid potential accidents, you can proceed with a new character. Since the cow elimination count works separately for each character, if you eliminate about 600 cows in Farobru and reach 666 in Hawezar or Scosglen, you are guaranteed to obtain the Bloody Wooden Shard and Musty Tome at the 666th kill. If you use this method, be sure to keep track of how many cows you have eliminated.
I used a single character; I obtained the Bloody Wooden Shard on the second kill in Hawezar and the Musty Tome after eliminating about 600 cows in Scosglen. Each icon marked below spawns 0–3 cows, but if there are no cows at the first location you visit after logging back in, it is highly likely they haven't respawned at the other locations either, so I recommend logging out and back in immediately.




Once you have obtained all three items, move to the pond in Ked Bardu and drop all three items. Each time you drop one, you can interact with one of the cow statues around the pond. Once you interact with all three statues, the Strange Key will drop in the middle of the pond.
With the Strange Key, you can enter the Lonely Shack at the eastern end of Scosglen. Inside the shack, you can see the corpses of slaughtered villagers and a herd of cows standing over them. After eliminating all the cows in the shack, you can obtain the Endurance Potion.



Second material, Rusted Bardiche
1. Obtain the Chatty Gem by using the 'Thanks' Emote in front of the 3 statues at Kurast Docks
2. Eliminate the 'Lacuni' elite monster in Ichorpol using only basic attacks to obtain the Bent Staff
3. Approach the 12 bird statues in Samuk village in order to obtain the Old Rusty Bell
4. Lure the Parasitized Zombies that spawn in Ichorpol to one place and eliminate them
5. Drop the Gem, Staff, and Bell on the mushroom patch that grew from the ground to contaminate them
6. Offer each item at the Okabo Temple brazier to obtain the Unusual Key
7. Move to the Lonely Cave west of Kurast, eliminate the monsters, and move to the hidden path
8. Eliminate the cows to obtain the Rusted Bardiche
The second step takes place in the region from Diablo 4's first expansion, 'Vessel of Hatred'. You must obtain various Quest items in this region that symbolize the materials for entering Whimsyshire in Diablo 3. First, move south of Kurast Docks and use the 'Thanks' Emote in front of the 3 statues to obtain the Chatty Gem.
Next, you must obtain the Bent Staff by eliminating an elite monster named 'Lacuni' north of Ichorpol using only basic attacks. To use basic attacks, you must reset your Skills and unequip all gear that has 'Skill Level' bonuses. If resetting and re-specing your skills is cumbersome, you can log in with a sub character to obtain it, just like with the 666 cow elimination.
Afterward, move to Samuk village, approach the bird statues in order, and return to the first bird statue to obtain the Old Rusty Bell. The bird statues are very small and are not interactable objects, so I recommend looking closely and getting as close as possible.






Afterward, you need to take the three items and eliminate three Parasitized Zombies that spawn north of Ichorpol. However, since the spawn locations of each zombie are quite far apart, you must lure them while only picking off other monsters, so please refrain from using AOE skills.
When you eliminate the three zombies, dark mushrooms will spread on the ground. If you drop the Chatty Gem, Bent Staff, and Old Rusty Bell you obtained earlier onto these mushrooms, purple smoke will rise and the items will be contaminated. Once you confirm the purple smoke, pick up the items again, move to the Okabo Temple located southeast of Kurast, offer them to each brazier, and obtain the Unusual Key.
Afterward, if you clear the 'Lonely Cave' basement west of Kurast, you can enter a hidden path. Inside, just like in the Lonely Shack, there are piles of villager corpses and cows. Once you eliminate all the cows, you can obtain the Rusted Bardiche.







Third material, Neyrelle's Hand
1. Check the Exile's Journal in the central cave of Scartara
2. Interact with the Sacred Cedar Tablet at the main gate of Kyovashad using 'Pride'
3. Check for text changes in the Exile's Journal in the central cave of Scartara
4. Destroy the ballista near Tyrael's Camp to obtain the Amazon Bolt
5. Enter the Idyllic Outskirts dungeon on Pilios Island
6. Interact with the uncharged ballista at the entrance to move to a hidden path
7. Interact with the statue at the end of the dungeon and eliminate the monster to obtain the Father's Chain
8. Interact with the beacon north of Pilios Island to burn the Father's Chain
The method to obtain Neyrelle's Hand is quite complex. Most of it takes place in the new expansion region, 'Vessel of Hatred'. First, check the contents of the Exile's Journal located in a small cave north of the Cataclysm dungeon in the center of Scartara, then move to Kyovashad. Select the 'Pride' option at the Sacred Cedar Tablet located at the entrance of Kyovashad, then return to the Exile's Journal to check for text changes.
Afterward, you can obtain the Amazon Bolt by destroying the ballista near Tyrael's Camp. Please be careful, as there is a bug where the Amazon Bolt does not drop for characters created before the release of the Vessel of Hatred expansion. Take the Amazon Bolt and move to the Idyllic Outskirts dungeon located southeast of Pilios Island.
As soon as you enter the dungeon, you will see an uncharged ballista; interacting with this object will fire a rope, allowing you to move to a new path different from the existing dungeon route. Follow the path along the rope, interact with the Inarius statue, and after eliminating the boss, you can obtain the Father's Chain item. Take the Father's Chain to the beacon located north of Pilios Island and burn it to complete the first step to obtaining Neyrelle's Hand.










1. Interact with the Sacred Cedar Tablet at the main gate of Kyovashad using 'Wrath'
2. Check for text changes in the Exile's Journal in the central cave of Scartara
3. Enter the Training Ground in Themis and hit the scarecrow to obtain the Amazon Training Helmet
4. Eliminate the Corrupted Amazon near Tyrael's Camp to obtain Amazon Training Equipment
5. Eliminate Vasha while wearing all the Amazon Training Equipment to obtain the Mother's Rose
6. Interact with the beacon north of Atuluwa to burn the Mother's Rose
Afterward, return to the Sacred Cedar Tablet at the entrance of Kyovashad, select the 'Wrath' option, and check for text changes in the Exile's Journal in the center of Scartara. Next, if you hit the scarecrow in the Training Ground located in Themis, you can obtain the Amazon Training Helmet.
Wear this Training Helmet and eliminate the Corrupted Amazons throughout the north of Celestia, including near the ballista where you obtained the Amazon Bolt, to have the Amazon Training Equipment drop in sequence. You must equip the dropped equipment immediately for the next item to drop, so be sure to equip it right away.
Additionally, I encountered a bug in this section where the Training Amazon Equipment stopped dropping halfway through, and many others have experienced the same bug. I couldn't identify the conditions that cause the bug, but if it occurs, you can log in with a sub character and proceed from the 'Wrath' option at the Sacred Cedar Tablet to obtain the Training Amazon Equipment. This equipment is shared within the account, so you can pass it to the character you were originally using for the quest to finish it. In my case, the same bug occurred up to the second and third characters, but except for the issue that obtaining the helmet takes a long time, the quest can be progressed even with a level 1 character, so if you are in a similar situation, you can create a new character to proceed.
Once you have obtained and equipped all five pieces of Amazon Training Equipment, eliminate the 'Vasha' elite monster that spawns east of Celestia, where Tyrael's Camp is located, to obtain the Mother's Rose. Once you have the rose, interact with the beacon north of Atuluwa to burn the Mother's Rose.









1. Interact with the Sacred Cedar Tablet at the main gate of Kyovashad using 'Fear'
2. Interact with the large raven on Lycander Island, then follow the raven to obtain the Mystic Priest's Skull
3. Interact with the beacon south of the Temple of Life to burn the skull and obtain the Deep Sea Key
4. Fish at the end of the shipwreck east of Hawezar to catch the Locked Wooden Coffin
5. Ride the wooden coffin to the Temple of Rathma
6. Interact with the bloated calf corpse at the end of the temple to obtain Neyrelle's Hand
Now for the final stage of Neyrelle's Hand. Interact with the Sacred Cedar Tablet at the main gate of Kyovashad and select the 'Fear' option. Afterward, you must find the large raven located south of Lycander Island. You have to diligently follow the flying raven; it may move along paths that players cannot traverse, so use your mount or movement skills as much as possible to ensure you don't lose it.
After circling the nearby area and arriving back at the original location, the raven will drop the Mystic Priest's Skull and fly off somewhere. If you burn this Mystic Priest's Skull at the beacon south of the Temple of Life, the Deep Sea Key will drop. Take the key and move east of Hawezar, where the shipwreck where Neyrelle's hand was severed in the campaign is located; you can fish up the Locked Wooden Coffin from the wooden planks at the end of this shipwreck.
If you open the wooden coffin with the Deep Sea Key and enter, you will be transported to the Temple of Rathma, which also appeared in the campaign. If you interact with the bloated calf corpse located at the end of the Temple of Rathma, you can finally obtain the last material, Neyrelle's Hand.









Final stage, entering the Cow Level
1. Combine the Endurance Potion, Rusted Bardiche, and Neyrelle's Hand in the Horadric Chest
2. Use the Loot of the Faithful to obtain the 'Moo' Emote and Torn Book Page
3. Interact with the Sacred Cedar Tablet at the main gate of Kyovashad using 'Greed'
4. Check for text changes in the Exile's Journal in the central cave of Scartara
5. Board the Exile's Ship located southeast of Pilios Island
6. Enter the Lonely Basement inside Sailara Island
7. Eliminate the cows that can only be attacked on Tuesday and enter the Secret Cow Level portal
If you have finished all these processes, you are finally ready to enter the Cow Level. If you combine the Endurance Potion, Rusted Bardiche, and Neyrelle's Hand obtained so far in the Horadric Chest, you can obtain an inventory item called Loot of the Faithful. If you use this inventory item, you can obtain the 'Moo' Emote and a Torn Book Page. Afterward, select the 'Greed' option at the Sacred Cedar Tablet at the main gate of Kyovashad and go to the central cave of Scartara to confirm that the text in the Exile's Journal has changed.
Immediately take the Torn Book Page and go southeast of Pilios Island to board the Exile's Ship. When you board the Exile's Ship, a cutscene parodying the opening of the campaign will play, and you will be transported to Sailara Island. There are minor Easter eggs hidden on Sailara Island. Representatively, you can see a skeleton horse stuck in a rainbow-colored puddle at the bottom of the island; if you fish while looking at this place, which parodies Whimsyshire from Diablo 3, you can catch a Mythic unique fish called 'Vivid Oddity'.
If you enter the Lonely Basement in the center of the island, you will see cows surrounding a magic circle. These cows can only be attacked from 9:00 AM Tuesday to 8:59 AM Wednesday (KST). Once you eliminate all the cows, a portal to the Secret Cow Level will open in the center.
If you want to visit the Cow Level again later, or if you want to bring a friend or acquaintance to the Cow Level, you do not need to repeat all these processes or visit Sailara Island on a Tuesday. If you return to the Sacred Cedar Tablet at the main gate of Kyovashad, you can see that the 'Moo' interaction has been added. If you select that option and go to the center of Scartara, you can see that a portal to the Secret Cow Level has been created in the cave where the Exile's Journal was.
If you use the 'Moo' Emote near a party member while in a party, the 'Moo' option will also be added to the Sacred Cedar Tablet for the party member, allowing them to open the portal, but please note that party members cannot clear the challenges related to the Cow Level.










In the Secret Cow Level, numerous bipedal cow monsters named 'Tormented Cow' appear, and you can also meet the traditional boss of the Cow Level, the 'Cow King'. When you eliminate the Cow King, a Mythic unique helmet called the Cow King's Crown and a 'Prime Rib' Quest item drop. The Cow King's Crown only has comedic options, so its performance isn't actually that good. It's a bit disappointing when you think of the Cow Level in Diablo 2 or Whimsyshire in Diablo 3, which were good farming spots, but as it is a Easter egg that requires a long time, it seems that if it were an overly efficient dungeon, complaints would have arisen among users.
Due to a bug in the middle, my pure playtime took nearly 20 hours, but if you proceed normally without any major bugs, it seems possible to achieve it within 10 hours or so. The 'Moo' Emote is cute, and it was a Easter egg that left me feeling proud as I had spent a long time on it. If you are a user who is wandering around because there is no proper content at the end of the season, why not try the Cow Level?
